Procedure to Become a Member
Membership of the Headache Society of India is applied for online through this website. The process is straightforward: complete the application form, upload your photograph and supporting documents, provide the names and membership numbers of two proposers who are Life Members of HSI, and pay the applicable subscription. Every application is then placed before the Governing Body for approval.
Complete the Online Membership Form
Open the membership form on the HSI website and select the category of membership you are applying for — Life Member, Associate Member or Associate Life Member. Enter your personal, academic and professional details in full.
- Full name, date of birth and gender.
- Correspondence address, city, state and PIN code.
- Mobile number and a working e-mail address, as all communication from the Society will be sent to these.
- Qualifications with year and institution, current designation and place of work.
- Medical council registration number.
Please check every entry before submitting. The name you enter will appear on your membership record and identity card exactly as typed.
Upload Your Photograph
Upload a recent passport-size colour photograph in JPG or PNG format. The photograph should have a plain background with the face clearly visible, and will be used for the Society's records and for your membership identity card.
Upload the Necessary Documents
Scanned copies of the following must be uploaded with the form. Scans should be legible and complete; unclear or partial uploads will delay processing.

Enter the Details of Your Two Proposers
Applications for Life, Associate and Associate Life membership must be recommended by two existing Life Members of the Headache Society of India. Please enter, in the space provided on the form:

Kindly obtain the consent of both proposers before entering their details. The Society may contact them to confirm the recommendation. An application submitted without valid proposer names and membership numbers cannot be processed.
Pay the Subscription
Pay the fee applicable to your membership category. Payment may be made through the online payment facility, or by cash, or by demand draft or crossed cheque drawn in favour of “Headache Society of India”. Upload the transaction reference or a scanned copy of the instrument along with your application.
Approval by the Governing Body
All completed applications are presented to the Society's Governing Body. Membership takes effect only upon approval by the Governing Body, in accordance with the Rules and Regulations of the Society.
- You will receive an acknowledgement by email once your application has been received.
- On approval, you will be informed in writing and enrolled in the register of members with your membership number.
- If an application is not approved, the reason for refusal will be communicated to the applicant concerned.
Life Members and Honorary Members acquire the right to vote at General Body Meetings one year after joining the Society. Associate Members and Associate Life Members enjoy all other privileges of membership but do not have voting rights; Associate Members may apply for Life Membership on successful completion of DM / DNB Neurology.
Before You Submit — A Short Checklist
- Online form completed in full, with all details verified.
- A recent passport-size photograph was uploaded.
- Degree certificates, medical registration certificate and address proof uploaded.
- Names and HSI membership numbers of both Life Member proposers entered, with their consent obtained.
- Subscription paid and proof of payment uploaded.
